Mass General Brigham Community Physicians, Inc. seeks a Patient Navigator to support patients at our Chestnut Hill practice, four days per week on-site. You will work with patients, families, and the healthcare team to ensure a seamless, patient-centered care experience.

A Bachelor's degree is required, with 1+ year in social work, community health, or case management. Proficiency with electronic health records and care coordination is preferred.

• Bachelor's Degree in related field required.

• Strong knowledge of healthcare resources and patient advocacy.
• Familiarity with electronic health records and case management software.
