Qualifications

• Bachelor's degree required.
• 1 year of related experience required.
• Licenses & Certifications: RN licensed in Illinois, OR Bachelor's/Master's in human services, OR LPN with 1 year elderly assessment experience.

Responsibilities

• Evaluate the needs of the member and recommend/facilitate plans for the best outcome.
• Assist with developing ongoing long-term care plans/service plans.
• Coordinate between the member and care provider team to ensure services are accessible.
• Provide resource support to members and their families/caregivers.
• Monitor care plans/service plans and provide recommendations based on member needs.
• Interact with long-term care healthcare providers to ensure member needs are met.
• Collect, document, and maintain long-term care member information.
• Perform on-site visits to assess member's needs.
• Provide education to long-term care members and their families/caregivers.
• Provide feedback to leadership on opportunities to improve care quality.
