Job summary

Qualifications

• Requires a Bachelor's degree and 1 year of related experience.
• For Illinois Plan: meet one of three criteria (RN IL license; or Bachelor's/Master’s in human services; or LPN with relevant experience).

Responsibilities

• Evaluates member needs and resources to recommend or facilitate optimal long-term care plans.
• Coordinates between members, families, and care providers to ensure access to services.
• Provides education to members and caregivers on procedures, options, and benefits.
• Documents and maintains LTSS information to ensure regulatory compliance.
• May perform on-site visits to assess needs and collaborate with providers.
