Job summary

Qualifications

• Bachelor’s degree in social sciences, social services, human services, behavioral sciences or an allied field and two years of experience providing social services to people with intellectual or developmental disabilities.
• Or six years of experience in providing social services to people with intellectual or developmental disabilities.
• Or 12 months as a Case Resource Manager Trainee.
• Or Equivalent experience/education.

Responsibilities

• Conduct functional and financial HRSN assessments using CARE Web to determine level of care and eligibility for the HRSN.
• Complete initial client assessments and person-centered service plans in CARE within established timelines as required for caseload and according to WAC and send appropriate notices to client and representative.
• Identify support needs of clients of DDCS and plan for and authorize payment for appropriate services.
• Obtain translation and interpretation services for clients and families when needed or requested.
• Refer individuals to appropriate DDCS services, other department services, and community programs/resources based on assessed needs.
• Identify clients or families in crisis and make emergent referrals to appropriate authorities, department/administration services, state plan services, or local community resources.
• Travel within region for in-person assessments and meetings; work in office with hybrid schedule; attend required trainings; participate in collective bargaining.
