Job summary

Washington State Department of Social and Health Services (DSHS) seeks a Resource Case Manager to provide specialized case management and resource assistance for people with developmental disabilities and their families in Yakima, WA.

The role emphasizes independence, informed choice, and quality of life, with home visits, crisis intervention, eligibility determinations, and coordination with state agencies. Hybrid work and travel within WA are expected.

Qualifications

• Bachelor’s degree in social sciences, social services, human services, behavioral sciences or allied field.
• Two years of social services experience with people with intellectual or developmental disabilities.
• Six years of social services experience.
• 12 months as Case Resource Manager Trainee.
• Graduate training can substitute for one year of experience.

Responsibilities

• Identify support needs of clients and authorize payments for services.
• Schedule and conduct face-to-face home visits with clients and families.
• Facilitate person-centered goal-planning discussions and update assessments/service plans.
• Complete and revise client assessments and service plans within timelines.
• Identify clients or families in crisis and refer to appropriate authorities or services.
• Determine and evaluate eligibility for programs.
• Maintain and document complete case files and payments.
