Qualifications

• Bachelor's degree in social sciences or allied field and two years of relevant experience.
• Or six years of experience in providing social services to people with intellectual or developmental disabilities.
• Or 12 months as a Case Resource Manager Trainee.
• Graduate training can substitute for one year of experience.

Responsibilities

• Identify client support needs and plan/authorize services.
• Schedule and conduct home visits with clients and families.
• Facilitate person-centered goal planning and update service plans.
