Job summary

The Developmental Disabilities Administration in Everett is hiring a Health-Related Social Needs Referral and Authorization Case Manager to support HRSN services for Apple Health clients.

This project position runs through June 30, 2028 and offers in-office, telework, or hybrid options. Typical hours are Monday–Friday, 8am–5pm, with travel for in-person assessments and partner meetings across the region.

Qualifications

• Bachelor's degree in social sciences or related field and two years of experience providing social services to people with intellectual or developmental disabilities; or six years of such experience; or 12 months as Case Resource Manager Trainee.
• Working knowledge of DD guidelines, RCWs, WACs, and related procedures.
• Ability to safeguard confidential information and report sensitive issues to supervisor.
• Ability to prioritize and organize work in a fast-paced environment.
• Sensitivity to cultural diversity.

Responsibilities

• Conduct functional and financial HRSN assessments using CARE Web to determine level of care and eligibility for HRSN.
• Complete initial client assessments and person-centered service plans within established timelines.
• Identify client support needs and plan/authorize payment for services.
• Arrange translation/interpreting services when needed.
• Refer individuals to DDA services or community resources as appropriate.
• Make emergent referrals when clients are in crisis.
