Job summary

The Developmental Disabilities Administration (DDA) in Lynnwood is hiring multiple Case Resource Managers to support individuals in residential settings. Roles involve in-home assessments, goal planning, and coordinating services with families and community partners.

Qualifications

• Bachelor's degree or six years of social services experience.
• Six years of social services experience with individuals with intellectual or developmental disabilities.
• 12 months as Case Resource Manager Trainee or equivalent.
• Graduate training may substitute for one year of experience.

Responsibilities

• Identify support needs of clients and plan for and authorize services.
• Schedule and conduct face-to-face home visits and living quarters with clients and families.
• Facilitate person-centered goal-planning discussions with clients and family.
• Complete and revise client assessments and service plans within timelines.
• Identify clients in crisis and make referrals to authorities or community resources.
• Determine financial and functional eligibility for programs.
• Review and monitor client plans and progress.
• Establish and maintain complete case files and payment records.
