Job summary

Pacific Health Group is seeking a Housing Stability Services professional to support Medi-Cal members by coordinating housing transitions, tenancy services, and deposits. You will perform telephonic outreach, conduct intakes, and collaborate with shelters, landlords, and providers to secure long-term housing stability.

The role includes fieldwork, independent caseload management (60–70 members), and adherence to state guidelines.

Qualifications

Strong understanding of the housing system and local housing resources or related workflows.
Experience in case management, social services, or healthcare.
Ability to manage a high-volume caseload independently.
Proficient in documenting in real-time and using multiple systems.

Responsibilities

Support members in housing transition and tenancy services.
Coordinate deposits and resources to remove financial barriers to housing.
Engage with housing authorities, shelters, landlords and community orgs.
Maintain high-quality documentation and ensure compliance with program requirements.
