Job summary

Pacific Health Group is seeking a field-based Housing Stability & Care Coordination Specialist in Santa Cruz County. You will support Medi-Cal members, guiding them through housing transitions and coordinating essential health and social services.

The role emphasizes direct community engagement, collaboration with housing authorities, and maintaining meticulous documentation to meet program requirements.

Qualifications

• 3–5 years in case management, social services, or healthcare.
• Experience with Medi-Cal, CalAIM, and Community Supports programs (preferred).
• Proficient with case management systems, EHR and documentation tools.

Responsibilities

• Support members directly in the community through housing stability services.
• Coordinate housing tenancy and sustaining services with stakeholders.
• Conduct comprehensive member assessments and develop housing care plans.
• Document interactions and ensure compliance with Medi-Cal, CalAIM, and Community Supports requirements.
