Job summary

Pacific Health Group in California is seeking a Housing Stability Services Coordinator to support Medi-Cal clients through housing navigation, tenancy services, and deposit coordination.

You will perform fieldwork as needed, manage a caseload of 60–70 members, and collaborate with housing providers, healthcare partners, and community organizations to ensure stable housing and service access. Strong documentation is essential.

Qualifications

• Strong understanding of the housing system and local housing resources.
• Experience: 3–5 years in case management or healthcare social services.
• Experience with Medi-Cal, CalAIM, and Community Supports programs (preferred).
• Ability to manage a high-volume caseload independently with strong documentation.

Responsibilities

• Provide housing stability services, including navigation, tenancy, and deposit assistance.
• Coordinate care across housing, healthcare, and community resources.
• Engage with housing authorities, landlords, shelters, and service providers.
• Maintain real-time documentation and ensure regulatory compliance.
