Qualifications

• 3–5 years in case management, social services, or healthcare.
• Experience with Medi-Cal, CalAIM, and Community Supports programs (preferred).
• Strong understanding of healthcare systems and community resources.
• Proficient in technology and real-time documentation.

Responsibilities

• Provide field-based housing navigation and care management.
• Coordinate housing deposits, tenancy, and sustained services.
• Lead comprehensive member assessments and care plan development.
• Engage with housing authorities, landlords, and providers.
• Document in real-time and maintain compliant records.
