Pacific Health Group is seeking a field-based Housing Stability Specialist to support CalAIM Community Supports. You will conduct intake, housing assessments, and navigation services, coordinating with housing providers and healthcare teams to secure stable housing and access to services.

Qualifications

• 3–5 years in case management, social services, or healthcare.
• Knowledge of Medi-Cal, CalAIM and Community Supports programs (preferred).
• Strong communication, empathy, and cultural competence.
• Proficient with technology and real-time documentation tools.
• Ability to document accurately and manage a caseload.

Responsibilities

• Provide Housing Transition Navigation and housing-related case management in the field.
• Coordinate Housing Tenancy & Sustaining Services and deposits with partners.
• Engage members, families, and providers to ensure stable housing and care.
• Perform outreach, assessments, and ongoing care coordination.
• Document encounters and progress in real-time per guidelines.
• Travel locally across counties and maintain compliance with regulations.
