Job summary

Pacific Health Group is seeking a field-based Housing Stability and Care Coordination professional. You will support Medi-Cal members through housing transition navigation, tenancy services, and deposits coordination while coordinating care with healthcare and community providers.

The role requires strong time management, leadership, and empathetic engagement. Field travel across Mendocino County and surrounding areas is expected, with a hybrid work setup and a focus on improving housing

Qualifications

• Experience in field-based community work or housing support programs.
• Strong communication and empathy with diverse populations.
• Ability to manage time and caseloads in a fast-paced environment.
• Proficiency with electronic health records and case management tools.

Responsibilities

• Provide Housing Transition Navigation and related housing support services.
• Coordinate tenancy, deposits, and ongoing housing case management.
• Engage with housing providers, shelters, landlords, and community partners.
