Pacific Health Group is seeking a field-based Housing Stability Specialist to support community members with housing transitions, tenancy sustainment, and deposit assistance. The role emphasizes independent, in-person work across Santa Cruz County and surrounding areas.

You will coordinate with housing authorities, shelters, and landlords, while maintaining real-time documentation and ensuring compliant service delivery within Medi-Cal and CalAIM frameworks.

Qualifications

• Strong understanding of the housing system and local housing resources.
• Experience in housing navigation and community-based support programs.
• Familiarity with Medi-Cal, CalAIM, and Community Supports workflows.

Responsibilities

• Provide Housing Transition Navigation and Housing Tenancy & Sustaining Services.
• Coordinate housing deposits and landlord engagements.
• Conduct home visits, assessments, and follow-ups in field settings.
• Coordinate with housing authorities, shelters, and providers to secure placements.
