Job summary

Pacific Health Group seeks a field-based Housing Stability Specialist to support Medi-Cal members in Monterey County. You will provide housing navigation, tenancy and deposits support, care coordination, and community outreach, while coordinating with housing authorities and providers to secure stable housing.

In a hybrid, non-manager role, you’ll conduct in-person visits, document in real time, and travel locally.

Qualifications

• Strong understanding of the housing system and local resources.
• Experience: 3-5 years in case management, social services, or healthcare.
• Proficiency with Medi-Cal, CalAIM, and Community Supports (preferred).
• Excellent communication, empathy, and cultural competence.
• Proficient with case management systems and EHRs.

Responsibilities

• Support members experiencing homelessness or at risk by providing Housing Transition Navigation Services.
• Coordinate Housing Tenancy & Sustaining Services for long-term stability.
• Assist with Housing Deposits and documentation for deposit assistance.
• Collaborate with housing authorities, landlords, shelters and providers.
• Document in real-time and maintain high-quality chart notes.
