Job summary

Pacific Health Group is seeking a field-based Housing Stability Specialist to support CalAIM Community Supports Housing programs and coordinate housing resources for high-needs members in a hybrid work model. You will conduct outreach, complete intakes, and build partnerships with housing providers and community organizations.

Qualifications

• Experience: 3-5 years in case management, social services, or healthcare.
• Proficiency with Medi-Cal, CalAIM and Community Supports programs is preferred.
• Ability to document in real-time and manage member data accurately.

Responsibilities

• Provide housing stability services and navigate housing resources.
• Coordinate with housing authorities, landlords, shelters, and community organizations.
• Deliver high-volume outreach and field-based case management to support 60-70 members.
• Maintain compliance and real-time clinical documentation.
