Job summary

Pacific Health Group is hiring for a field-based Housing Stability Services role in California. You will support Medi-Cal members, coordinate housing resources, and provide end-to-end care management with a focus on housing as a key driver of health.

The position is hybrid (California remote with occasional field work) and involves a caseload of 60–70 high-need members, requiring strong organization, independence, and timely documentation.

Qualifications

• 3–5 years in case management, social services, or healthcare.
• Proficient with Medi-Cal, CalAIM, and Community Supports programs (preferred).
• Experience independently managing an active caseload in care coordination.
• Strong understanding of healthcare systems, managed care, and community resources.
• Excellent communication, empathy, and cultural competence.

Responsibilities

• Support members in CalAIM Community Supports Housing programs and housing services.
• Conduct telephonic outreach and comprehensive intakes to identify housing needs.
• Build partnerships with housing providers, shelters, landlords, and community organizations.
• Provide person-centered housing care management to high-needs members.
• Occasional fieldwork delivering face-to-face case management across shelters and residences.
• Manage the Housing resources pipeline and deposit assistance documentation.
• Engage in community outreach to build referral routes.
• Maintain high-volume telephonic/field outreach with real-time documentation.
• Manage caseload of 60–70 members needing housing stability support.
• Ensure compliance and timely documentation.
