Pacific Health Group seeks a dedicated housing stability and care coordination professional to support CalAIM Community Supports housing initiatives in a Hybrid CA setting. You will conduct field-based care coordination, housing assessments, and partnerships to secure stable housing for members.

The role emphasizes independent caseload management, timely documentation, and collaboration with housing authorities and community partners to drive housing access and health outcomes.

Qualifications

• 3–5 years in case management, social services, or healthcare.
• Strong understanding of housing systems and local housing resources.
• Experience with Medi-Cal, CalAIM, and Community Supports programs (preferred).
• Independent management of an active caseload in care coordination or housing navigation.

Responsibilities

• Support members experiencing homelessness or at risk of homelessness with housing transition navigation services.
• Coordinate housing tenancy and sustaining services with ongoing case management and landlord engagement.
• Assist members with housing deposits and documentation for access to deposit assistance.
• Collaborate with housing authorities, shelters, and community organizations for housing placement and tenancy.
• Maintain high-volume outreach, intake assessments, care plan development, and timely documentation.
