Pacific Health Group is seeking a dedicated housing stability coordinator to support Medi-Cal members through housing transition navigation and tenancy services. The role blends telephonic outreach with occasional field visits to housing sites and shelters, delivering person-centered care plans and real-time documentation.

Qualifications

• 3–5 years in case management, social services, or healthcare.
• Strong understanding of Medi-Cal, CalAIM, and Community Supports programs (preferred).
• Experience independently managing an active caseload in a care coordination, housing navigation, or case management environment.
• Excellent communication, empathy, and cultural competence.
• Proficient with case management systems, EHR and documentation tools.
• Must be able to travel within hiring county and surrounding counties.

Responsabilités

• Support housing program participants through Housing Transition Navigation Services.
• Coordinate Housing Tenancy & Sustaining Services.
• Assist members with Housing Deposits and coordinating required documentation.
• Collaborate with housing authorities, landlords, shelters, and service providers.
• Monitor housing stability and address barriers that could affect outcomes.
• Manage high-volume telephonic/field outreach and care plans with real-time documentation.
