Job summary

Pacific Health Group is seeking a Housing Stability Services Coordinator to support Medi-Cal members in securing and maintaining permanent housing. You will coordinate housing navigation services, deposits, and sustained tenancy while collaborating with shelters, landlords, and healthcare providers.

The role involves telephonic outreach, in-person fieldwork as needed, and real-time documentation to ensure compliant care delivery.

Qualifications

• 3–5 years of experience in case management, social services, or healthcare.
• Strong understanding of housing systems and local resources.
• Excellent communication, empathy, and cultural competence.
• Proficiency with case management systems, EHRs, and real-time documentation.

Responsibilities

• Support housing stability through navigation and services.
• Coordinate housing deposits, tenancy, and sustaining services.
• Engage with community organizations to expand access to resources.
• Document in real-time and ensure compliance with program requirements.
• Manage a caseload of 60–70 members requiring end-to-end care coordination.
