Job summary

Pacific Health Group is seeking a Housing Stability Services Specialist to support Medi-Cal members through housing navigation, care coordination, and community outreach. You will build partnerships with landlords, shelters, and providers while guiding members to stable housing and access to healthcare and social services.

The role includes occasional fieldwork and real-time documentation to ensure compliant, person-centered care.

Qualifications

• Strong understanding of the housing system and local resources.
• Experience with housing navigation and community supports programs.
• Ability to document in real time with accuracy.
• Proven ability to manage a high-volume caseload independently.

Responsibilities

• Support housing stability through navigation services and tenancy support.
• Coordinate Housing Deposits and documentation for access to deposit assistance.
• Engage with community partners to expand housing access and referrals.
• Conduct telephonic outreach and comprehensive intake assessments.
• Deliver field visits for housing assessments and in-person outreach as needed.
• Maintain compliant, high-quality case notes and care plans.
