Job summary

Pacific Health Group is seeking a Field-Based Housing Specialist to support Medi-Cal members in Monterey County. This role focuses on housing stability, care coordination, and navigation across housing resources while working in the community.

You will conduct assessments, develop individualized care plans, coordinate with landlords and providers, and document interactions in real time following state guidelines; this is a hybrid, non-exempt position with hourly pay of $30-$35.

Qualifications

• 3-5 years in case management, social services, or healthcare.
• Experience with Medi-Cal, CalAIM, and Community Supports programs (preferred).
• Strong understanding of healthcare systems and community resources.

Responsibilities

• Provide housing stability services and case management to high-needs members.
• Coordinate housing deposits and tenancy services with landlords.
• Build partnerships with housing providers, shelters, and community organizations.
• Conduct in-person visits, assessments, and follow-ups.
• Document in real-time per state guidelines.
