Job summary

Pacific Health Group is seeking a field-based Housing Stability & Care Coordination Specialist to support Medi-Cal members in housing pathways within California. You will navigate housing resources, coordinate tenancy services, and manage a caseload with a focus on durable housing solutions.

You will conduct intakes, collaborate with housing providers, shelters, and healthcare partners, and document progress in real-time while ensuring regulatory compliance.

Qualifications

• 3–5 years in case management, social services, or healthcare.
• Experience with Medi-Cal, CalAIM, and Community Supports programs (preferred).
• Proficient in case management systems and real-time documentation.

Responsibilities

• Support Housing Transition Navigation and Housing Tenancy & Sustaining Services.
• Coordinate Housing Deposits and documentation to remove financial barriers to housing.
• Provide comprehensive housing care management to high-needs members.
• Perform high-volume outreach (telephonic/field) and initial assessments.
• Maintain accurate and timely documentation compliant with state guidelines.
