Job summary

Pacific Health Group in Monterey County is seeking a Community Housing Navigator to support field-based housing stability services, coordinate transitions, and connect members to housing resources.

Qualifications

• Experience delivering housing navigation services in community settings.
• Ability to work independently in field and remote environments.
• Strong time management and leadership capabilities.

Responsibilities

• Support members experiencing homelessness by providing Housing Transition Navigation Services.
• Coordinate Housing Tenancy & Sustaining Services and Housing Deposits.
• Build partnerships with housing authorities, landlords, shelters, and social services to secure housing.
• Deliver comprehensive, person-centered housing care management and document in real time.
• Travel locally within Monterey County and surrounding counties.
