Job summary

Pacific Health Group is seeking a field-based Housing Stability Specialist to support Medi-Cal members through housing navigation and care coordination. You will work directly with members in the community, coordinating housing services and healthcare connections to promote long-term stability.

The role requires strong independence, time management, and hands-on engagement in field settings across Mendocino County. Travel locally within the county is expected, with a hybrid work arrangement.

Qualifications

• Strong understanding of the housing system and knowledge of local housing resources, housing navigation, and community supports.
• Experience in case management, social services, or healthcare settings is preferred.
• Proficient in documentation, EHR platforms, and real-time note-taking.

Responsibilities

• Provide Housing Transition Navigation Services to support stable housing.
• Coordinate Housing Tenancy & Sustaining Services with landlords and providers.
• Assist with Housing Deposits and document intake and deposits.
• Build partnerships with housing authorities, shelters, and community organizations.
• Travel locally within Mendocino County to engage members and assess needs.
