Job summary

Pacific Health Group is seeking a field-based Housing Stability Specialist to support Medi-Cal members in housing transitions and ongoing tenancy services. You will conduct housing intake, build relationships with shelters, landlords, and community providers, and coordinate healthcare and social services to promote housing stability.

The role requires 60–70% field travel within the county, strong time management, and the ability to work independently in fast-paced community settings while

Qualifications

• 3–5 years in case management, social services or healthcare
• Experience with Medi-Cal, CalAIM, and Community Supports programs (preferred)
• Strong understanding of healthcare systems, managed care, and community resources
• Excellent communication, empathy, and cultural competence
• Strong organizational and time management skills
• Proficient with technology, including case management systems and documentation tools

Responsibilities

• Support housing stability through Housing Transition Navigation Services and Housing Tenancy & Sustaining Services
• Coordinate housing deposits and assist with required documentation
• Build partnerships with housing authorities, landlords, shelters, and community organizations
• Conduct comprehensive member assessments and develop individualized care plans
• Travel locally within the county 60–70% for in-person visits
• Documentation of interactions in real-time following state guidelines
• Engage with community outreach events and partnerships
• Ensure continuity of care and timely coordination across platforms
• Provide field-based support and advocacy for high-needs members
