Job summary

Pacific Health Group seeks a field-based Housing Stability Specialist in California to support CalAIM housing programs and provide end-to-end care coordination for high-needs members. This hybrid role includes telephonic outreach, intakes, and occasional in-person fieldwork across shelters and housing sites.

You will collaborate with housing providers, landlords, and community organizations to improve access to stable housing while managing a dynamic caseload with documentation in real-time.

Qualifications

• Ability to document in real-time and manage member data accurately.
• Experience independently managing an active caseload in care coordination or housing navigation environment.
• Comfortable navigating multiple systems and communication platforms.

Responsibilities

• Coordinate appointments, services, and long-term housing support systems.
• Ensure continuity of care and consistent follow-through.
• Maintain accurate and timely documentation of all member interactions.
