Job summary

ServiceSource Inc. in Oakton, VA is seeking a Case Manager (Employment Specialist) to help individuals and families move toward self-sufficiency by linking them to employment opportunities, training resources, and wraparound supports.

The role is hybrid with up to 40 hours per week and a two-year funded contract. You will build relationships with employers and community partners, develop person-centered plans, and coordinate housing stabilization and related services to enable long-term

Qualifications

• Bachelor’s degree preferred.
• Associate degree in Psychology, Human Services, or related field with 1 year of relevant experience OR high school diploma/GED with 3+ years of human services experience.
• Minimum 1 year of supported employment experience, including job development, job coaching, vocational support, or workforce services.

Responsibilities

• Provide individualized, community-based employment and case management services to MTW households, including regular participant contact, ongoing engagement, progress monitoring, and support toward self-sufficiency goals.
• Complete comprehensive assessments and develop, implement, and update person-centered service plans addressing strengths, needs, employment objectives, housing stability, and independence.
• Deliver employment-related supports, including career exploration, employer outreach, interview preparation, onboarding, and follow-along services to promote job placement and retention.
• Coordinate housing stabilization supports and facilitate referrals and warm handoffs to training, benefits, childcare, transportation, housing, education, employment, and other resources.
• Collaborate with FCRHA staff, ServiceSource teams, community providers, and stakeholders to ensure coordinated service delivery and accurate documentation.
