Job summary

ServiceSource in Oakton, VA is seeking a Case Manager (Employment Specialist) to help individuals and families move toward self-sufficiency by connecting them with employment opportunities, training resources, and wraparound supports.

The role combines direct participant work with coordination among employers, training providers, and community partners across Fairfax County, including housing stability and long-term independence planning.

Qualifications

  • Bachelor’s degree preferred.
  • Associate degree in Psychology, Human Services, or a related field with 1 year of relevant experience OR high school diploma/GED with 3+ years of human services experience.
  • Minimum 1 year of supported employment experience, including job development, job coaching, vocational support, or workforce services preferred.
  • Experience building partnerships with employers and/or training providers; sales, marketing, or employer outreach experience preferred.
  • Knowledge of case management practices, documentation requirements, and service coordination.
  • Availability and willingness to travel weekly within the local area to perform the job; flexible hours including evenings and/or weekends when needed.
  • Valid driver’s license and reliable transportation to perform work-related travel.

Responsibilities

  • Provide individualized, community-based employment and case management services to MTW households, including regular participant contact, ongoing engagement, progress monitoring, and support toward self-sufficiency goals.
  • Complete comprehensive assessments and develop, implement, and update person-centered service plans that address participant strengths, support needs, employment objectives, housing stability, and long-term independence.
  • Deliver employment-related supports, including career exploration, employer outreach, interview preparation, hiring and onboarding assistance, and follow-along services to promote job placement and retention.
  • Coordinate housing stabilization supports and facilitate referrals and warm handoffs to training, benefits, childcare, transportation, housing, education, employment, and other community-based resources.
  • Collaborate with FCRHA staff, ServiceSource teams, community providers, and other stakeholders to ensure coordinated service delivery and maintain accurate, timely documentation of services, referrals, participant progress, and outcomes.
  • Build and maintain long-term relationships with community partners, participants, their families, and other stakeholders to ensure access to up-to-date resources and support.
  • Regularly communicate and collaborate with FCRHA support staff.
  • Stay informed on current and emerging community resources, including programs and services related to healthcare, housing, education, employment, and other relevant supports.
  • Build and maintain partnerships with local organizations to ensure access to the most up-to-date information, enabling effective referrals and support for individuals and families.
  • Record participants’ progress including charting referrals, home visits and other notable interactions.
  • Facilitate and participate in team meetings.
  • Identify and/or provide support resources related, but not limited to independent living, aging in place, medical support, mental health support, emergency response plans/support, in-home companion care, housing, social and leisure activities, transportation, and financial planning and needs.
  • Actively seek opportunities to enhance knowledge of systems supporting FCRHA recipients, including the barriers they face. Foster collaboration through participation in community meetings, professional networks, and relevant trainings, enabling effective referrals and comprehensive care for individuals and families.
  • Regularly meet with the Director of Employment Services and key team members, including case managers, youth transition specialists, and employment development specialists from ServiceSource and affiliated organizations, to ensure alignment and progress toward grant objectives.
  • Perform other responsibilities as assigned.
