ServiceSource in Oakton, VA, is hiring a Case Manager (Intake Coordination) to support Fairfax County families on housing stability and self-sufficiency. The role offers a hybrid schedule, weekday hours, and a meaningful, predictable routine with no evenings or weekends.

You will conduct intake and assessments, identify barriers, connect clients with community resources, coordinate referrals, and develop person-centered plans focused on employment and independence.

Qualifications

• Bachelor’s degree preferred in social work, psychology, or human services.
• Experience with vulnerable populations and intake/eligibility coordination.
• Ability to conduct structured interviews, assessments, and document findings.

Responsibilities

• Schedule and conduct participant intake and enrollment and complete baseline assessments.
• Facilitate smooth transitions to ongoing case management and ensure follow-up.
• Coordinate services with partners to support participant success.
• Develop, implement, and update person-centered plans focused on self-sufficiency and employment goals.
• Connect participants to community resources and maintain accurate documentation.
