Job summary

ServiceSource, Inc. is seeking a dedicated Case Manager to support disconnected youth and adults with disabilities in a multi-year grant-funded program.

You will assess needs, create family-centered plans, and connect participants to essential community resources to achieve self-sufficiency and competitive employment. Responsibilities include coordinating referrals, updating care plans, and collaborating with employment development specialists and TANF case managers.

Qualifications

• Bachelor's degree in a relevant field is required.
• 2–5 years of experience working with individuals with disabilities and TANF is preferred.
• 2–5 years in case management, care coordination, and development/implementation of individualized plans; background checks and licenses required.

Responsibilities

• Conduct comprehensive assessments to identify support needs and establish goals with participants and families.
• Develop and update person-centered care plans aligned with employment goals.
• Link participants to community resources and guide navigation of complex systems.
• Monitor needs and address barriers to access and employment pathways.
