Job summary

ServiceSource in Oakton, VA is seeking a Case Manager (Intake Coordination) to support Fairfax County families on housing stability and self-sufficiency in a hybrid, weekday schedule.

You will conduct participant intakes, assessments, and connect clients with community resources, while developing participant-centered plans and coordinating referrals and services toward employment goals.

Qualifications

• Bachelor’s degree in social work, psychology, or human services preferred.
• 3–5 years of experience in case management, intake and eligibility coordination, care coordination, and development/implementation of individualized plans preferred.
• Valid driver’s license and willingness to travel within local area.

Responsibilities

• Schedule and conduct participant intake and enrollment, complete baseline assessments and identify needs.
• Facilitate smooth transitions to ongoing case management and ensure timely follow-up.
• Coordinate services with internal and external partners to support participant success.
• Develop, implement, and update person-centered plans focused on self-sufficiency and employment goals.
• Record progress and maintain documentation to support compliance and outcomes.
