Job summary

ServiceSource, Inc. seeks a dedicated Case Manager to support youth and adults with disabilities in a multi-year grant-funded program transitioning participants to competitive employment. You will build care plans, coordinate referrals, and navigate barriers alongside a collaborative team.

The role emphasizes wrap-around supports, ongoing plan updates, and partnership with TANF and education professionals. The position ends in early 2029 and requires strong organization and community resource

Qualifications

• Bachelor's degree in social work, psychology, or human services required; master's preferred.
• 2–5 years experience with individuals with disabilities and in case management.
• Valid state driver's license or reliable transportation.
• Background checks and other screenings as required.

Responsibilities

• Conduct comprehensive assessments to identify support needs and establish individualized/family goals.
• Develop and update person-centered plans aligned with employment goals.
• Link participants to community resources and assist with navigating systems.
• Monitor needs and address barriers to access services.
