ServiceSource is seeking a Case Manager to support Fairfax County households toward housing stability, financial independence, and long-term self-sufficiency. The role involves assessments, individualized service plans, and wraparound supports across housing, employment, and benefits, with a focus on coordination and community partnerships.

Qualifications

• Bachelor’s degree in social work, psychology, or human services required.
• Master’s degree preferred.
• 2-5 years experience with vulnerable populations (TANF, subsidized housing, homelessness, immigrants, or justice-involved).
• 2-5 years experience in case management, care coordination, and plan development.
• Knowledge of Fairfax County housing subsidies and related programs.

Responsibilities

• Complete comprehensive assessments and develop ITSPs based on participant needs.
• Provide participant-centered case management with regular engagement and progress reviews.
• Develop and update person-centered plans reflecting changing needs and goals.
• Coordinate housing stabilization and collaborate with housing navigators and employment specialists.
• Facilitate referrals and maintain timely documentation for compliance and reporting.
• Build long-term relationships with partners, participants, and stakeholders.
