Job summary

ServiceSource Inc. is seeking a Case Manager (Housing Navigation) to support Fairfax County families in securing and maintaining stable housing. The role involves housing searches, relocation planning, lease-up support, and coordinating with landlords, housing authorities, and community partners.

Hybrid schedule in Oakton, VA with a 9:00 a.m.-5:00 p.m. workday, emphasis on wraparound services, client-centered planning, and collaboration to help families achieve self-sufficiency.

Qualifications

• Bachelor's degree (B.A.) in human services or other related field and at least one year of relevant housing navigation experience; or Associate's degree in relevant field and at least three years of relevant experience preferred.
• Experience providing housing navigation, eviction prevention, or housing stability services for HUD-assisted or voucher-based households, including coordination with landlords and housing authorities preferred.
• Familiarity with Fairfax County housing market required.
• Experience working with FCRHA programs strongly preferred.
• Knowledge of case management practices, documentation requirements, and service coordination.
• Demonstrated ability to conduct structured interviews, complete assessments, identify immediate needs, and document findings accurately.
• Annual criminal background checks, child abuse clearances, and other required screenings in compliance with ServiceSource, state, and funding agency regulations.
• Availability and willingness to travel weekly within the local area to perform the job.
• Availability and willingness to work flexible hours, including evenings and/or weekends, when needed.
• Valid driver's license and/or access to reliable transportation to perform work-related travel required. Eligible drivers must have a good motor vehicle record (MVR).

Responsibilities

• Provide housing navigation and relocation support, including housing searches, affordability planning, move-in coordination, and lease-up support.
• Deliver participant-centered case management through assessments, monthly engagement, goal planning, and ongoing review of progress toward self-sufficiency.
• Develop and update individualized, person-centered plans addressing employment, housing stability, family support needs, and long-term independence.
• Coordinate closely with FCRHA, housing navigators, Case Managers, EDSs, and community partners to align services with ITSP goals, MTW objectives, and wraparound support needs.
• Connect participants to internal and external resources, referrals, training, benefits, childcare, transportation, employment, education, housing supports, and maintain accurate service documentation.
