Job summary

Foundation Communities is seeking a dedicated professional to provide intensive case management for residents of permanent supportive housing. You will collaborate with an interdisciplinary team to connect residents with mental health, primary care, financial literacy, education and employment services.

The role emphasizes outreach, crisis de‑escalation, and person-centered approaches to improve housing retention and self-sufficiency among adults facing homelessness or at risk.

Qualifications

• At least one year of experience providing intensive case management to adults experiencing homelessness or at risk of homelessness.
• Experience in mental health/substance use and crisis intervention required.
• Master of Social Work degree recommended.

Responsibilities

• Conduct intensive case management and care coordination to increase residents’ access to health and housing resources.
• Effectively assess and diffuse crisis situations using de-escalation techniques.
• Develop outreach strategies to build relationships and connect residents to services.
• Collaborate with Property Management to support safety and community well‑being.
• Document services in accordance with grant requirements and FC systems.
• Maintain case notes and reporting with timely submission when required.
