Job summary

Catholic Community Services seeks a Permanent Supportive Housing Case Manager to deliver intensive, client-centered services for individuals and families experiencing homelessness. You will use housing-first and trauma-informed approaches to promote self-sufficiency.

You will conduct assessments, develop housing stability plans, coordinate with healthcare, benefits, and employment services, and work with property management to maintain safe housing environments.

Qualifications

• Minimum 5 years’ experience in Social Services with 1-year experience in Case Management related to high needs, low-income families who have experienced housing instability.
• 4-years’ experience in Human Services or Social Services performing duties aligned with the major duties and responsibilities of the role.
• Associates Degree in Social Services, Psychology, Human Services, or related field with 1-year case management experience.
• Must pass background checks, provide three references, have reliable transportation, valid driver's license, insurance, and driving record per CCS policy.
• TB test negative or within past 12 months.

Responsibilities

• Case Management & Service Delivery: conduct assessments and develop Housing Stability Plans with measurable goals.
• Housing Stability Support: post housing availability, ensure lease compliance, educate on tenant rights, prevent evictions, coordinate with property management.
• Documentation & Compliance: maintain confidential records, complete HMIS documentation.
• Collaboration & Coordination: work with internal teams and external partners, participate in case conferences.
• Crisis Intervention: respond to crises using trauma-informed practices and connect to emergency services when needed.
