Job summary

TemPositions Group of Companies is seeking a Supportive Housing Case Manager to assist chronically homeless adults in a permanent supportive housing program in New York. You will connect residents to community resources and coordinate individualized services to promote stability, independence, and well-being.

This role requires experience with chronically homeless individuals, including those with Serious Mental Illness and co-occurring diagnoses.

Qualifications

• Master's in Social Work preferred or Bachelor's in related field required.
• At least 5 years’ experience with homeless or high-needs populations.
• Knowledge of community resources and housing systems; strong case management practices.
• Excellent communication, organizational and problem-solving skills.
• Experience with diverse and marginalized populations.

Responsibilities

• Conduct client intake, assessments, and individualized service planning.
• Assist clients with housing search, placement, and retention support.
• Provide ongoing case management, including referrals and progress monitoring.
• Respond to housing crises and coordinate with emergency or support services.
• Collaborate with healthcare and behavioral health providers.
• Advocate for client needs with landlords, agencies, and service systems.
• Maintain accurate records and required documentation.
