Job summary

Housing Works Inc. is seeking a Housing Case Manager to provide comprehensive case management and care coordination for residents living with HIV and/or co‑occurring conditions. You will visit clients in their homes, offices, and community settings to promote housing stability and independent living skills.

The role emphasizes facilitating assessments, care plans, referrals, and collaboration with multiple departments to meet residents’ health, housing, and social needs.

Qualifications

• Bachelor's degree in social work, psychology, counseling, or related field preferred.
• Minimum of three years of relevant social services experience with homelessness, mental health, or substance use.
• Proficiency in documenting services in electronic systems.

Responsibilities

• Provide comprehensive case management and care coordination for residents living with HIV and/or co-occurring conditions.
• Conduct regular home visits and assessments; develop and update service plans.
• Maintain accurate documentation and comply with funder and regulatory standards.
• Coordinate care with primary care, behavioral health, and community partners.
• Support residents’ housing stability, independence, and wellness.
