Job summary

ServiceSource Inc is seeking a dedicated case manager to support residents with disabilities and low-income households. The role emphasizes person-centered planning, crisis intervention, and collaboration with housing staff to promote independent living and tenancy success.

You will maintain confidential case notes, prepare referrals, and coordinate with healthcare, transportation, and legal resources to enhance access to essential services in a quiet, professional environment.

Qualifications

• Bachelor’s degree in Social Work, Psychology, Business, or related field.
• Licensed Social Worker (LSW) strongly preferred.
• At least one year of case management experience.
• Valid driver’s license and good motor vehicle record.

Responsibilities

• Conduct resident intake, assessments, and ongoing needs evaluations for low-income residents, including individuals with disabilities and residents facing barriers to housing stability, healthcare access, transportation, employment, benefits, or daily functioning.
• Develop individualized service plans identifying goals, barriers, strengths, referrals, and measurable steps to support independent living and successful tenancy.
• Provide direct case management, crisis intervention, resident advocacy, and supportive counseling within scope of practice to assist residents with problem-solving and access to services.
• Maintain accurate, timely, and confidential case notes, referral records, service plans, outcomes tracking, and program documentation.
• Coordinate referrals to healthcare, behavioral health, disability services, benefits counseling, transportation, legal aid, food access, vocational support, and other services.
