Job summary

Project Home is seeking a Case Manager to connect, coordinate, and monitor community resources for adults and families in need of housing support. You will assess needs, develop care plans, and guide participants toward independence within agency standards.

Under supervision of the Program Coordinator, you will coordinate with outside agencies, maintain records, and monitor services to ensure effectiveness. Some duties may occur in clients’ homes and community settings.

Qualifications

• Knowledge on mental health and mental health treatment services preferred.
• Knowledge on homelessness and supportive housing preferred.
• Ability to effectively connect and work with participants while maintaining boundaries.
• Ability to schedule one's times and activities efficiently while prioritizing duties.
• Ability to maintain patience, confidence, and composure under pressure or adverse circumstances.

Responsibilities

• Complete supportive housing case assessments under supervision.
• Develop care plans to stabilize housing for adults and families.
• Coordinate services with outside agencies and monitor progress.
• Maintain client case records and maximize reimbursements.
• Participate in professional development activities.

Behavioral Health Practitioner standards (DHS)

30 semester hours / 45 quarter hours in behavioral health

2000 hours supervised experience
