Job summary

Christian Community Health Center is seeking a Housing Case Manager to assist supportive housing clients in maintaining housing and increasing self-sufficiency through resource linkages and goal attainment. The role requires a Bachelor’s degree with social services experience and a valid driver’s license.

The position covers case management duties, home visits, service planning, and coordination of supportive services to improve housing stability.

Qualifications

• Bachelor’s degree plus two (2) years experience in social services (case management preferred).
• Experience serving homeless, ex-offenders, individuals living with HIV/AIDS and/or substance abuse populations preferred.
• A valid driver’s license with clean MVR is required or a clean driving record within the past 3 years.

Responsibilities

• Conducts a minimum of one monthly face-to-face contact per client.
• Provides a minimum of one monthly home visit per client or as needed.
• Completes chronological progress notes within three days of contact (DAP format).
• Develops and revises a client-specific service plan every six months or as needed.
• Assists no-income clients to identify resources to obtain entitlements within six months of admission.
• Documents referrals, outcomes, and maintains client files in Client Track within three days of encounters.
• Completes monthly statistical reports and participates in required trainings.
