Job summary

Mid-Willamette Valley Community Action Agency seeks a Case Manager to provide direct services to chronically homeless individuals, including eligibility screening, enrollment, and ongoing follow-up. You will conduct home visits, develop Individual Service Plans, and connect clients with housing resources and support services.

The role emphasizes collaboration with community partners, data integrity, and compliance with program guidelines across Housing First initiatives.

Qualifications

• High School Diploma or GED with two years of related social service experience, or equivalent.
• Ideal candidates have a college degree or specialized training with two+ years homeless service delivery experience.
• Knowledge of coordinated entry, unsheltered populations, and homeless service best practices preferred.

Responsibilities

• Facilitates access to rapid re-housing and barrier removal funds.
• Assess client eligibility and gather intake documentation and financial assistance information.
• Provide direct case management and housing placement support to promote self-sufficiency.
• Develops Individual Service Plans identifying needs, barriers and strengths.
• Conducts monthly client home visits as part of case management.
• Maintain relationships with community partners and ensure data quality and confidentiality.
