Job summary

Qualifications

• Minimum 5 years’ experience in Social Services.
• 1-year experience in Case Management with high needs families.
• Knowledge of behavioral health challenges and social services.

Responsibilities

• Provide benefit and resource case management.
• Respond to referrals and assess client needs.
• Create case plans with clients.
