This position for a Resource and Community Life Case Manager focuses on providing case management for clients in Permanent Supportive Housing at Catholic Community Services. Responsibilities include assisting families, connecting them to resources, and facilitating group activities.

The candidate should have at least 5 years of relevant social services experience or an Associate's degree in a related field, with knowledge of community resources and case management practices.

Qualifications

• Minimum 5 years’ experience in Social Services.
• 1-year experience in Case Management with high needs families.
• Knowledge of behavioral health challenges and social services.

Responsabilités

• Provide benefit and resource case management.
• Respond to referrals and assess client needs.
• Create case plans with clients.
