Job summary

Community Resource Center is hiring a Social Services Case Manager to provide housing-focused, participant-centered case management and supportive services. You will assess needs, develop individualized plans, and connect clients to resources to support housing stability.

Qualifications

• Associate degree in Social Work or related field required; BA/BS preferred.
• 1+ years in social services, case management or related field.
• Bilingual English/Spanish preferred.
• Ability to work independently in office and community settings; strong boundaries and judgment.

Responsibilities

• Provide housing-focused, participant-centered case management and supportive services.
• Complete intake, enrollment, and eligibility documentation; develop individualized case plans.
• Engage participants with flexible, client-centered outreach (in-office, telework, community-based).
• Conduct home visits to support housing stabilization and connect to resources.
• Travel locally to meet participants and attend off-site meetings and appointments.
• Maintain accurate records in agency databases and documentation standards.
• Collaborate with landlords, healthcare, and community partners to address barriers to housing stability.
• Participate in supervision, trainings, and team meetings as required.
