Job summary

Foundation Communities is seeking a highly motivated professional to provide intensive case management services to residents in permanent supportive housing communities, helping them maintain housing and access health, financial, education, and employment resources. You will work with an interdisciplinary team to promote housing stability and community well-being.

Qualifications

• At least one year of experience providing intensive case management services to adults experiencing homelessness or at risk of homelessness.
• Experience in mental health/substance use and crisis intervention.
• Ability to work independently with diverse backgrounds.
• Strong interpersonal, organizational, and multitasking skills.

Responsibilities

• Conduct intensive case management and care coordination to increase access to mental and primary health care, financial management, and self-sufficiency.
• Assess and diffuse crisis situations using de-escalation techniques and document incidents.
• Develop strategies to engage residents and connect them to resources, including groups and learning opportunities.
• Collaborate with Property Management to enforce policies and support community safety.
• Document delivered services in accordance with grant requirements and FC systems.

Education

Master of Social Work Degree (or related field) recommended
