Commonwealth Catholic Charities (CCC) seeks a Housing Case Manager to provide comprehensive services to individuals experiencing chronic homelessness, including those with addictions and mental illness. You will collaborate with participants to ensure housing stability and connect them with community resources.

Responsibilities include creating service plans, coordinating with agencies, assisting with long-term housing goals, processing paperwork, and reporting, while upholding CCC's

Qualifications

• Bachelor’s Degree with two years of related experience or equivalent education and experience.
• Valid driver’s license.
• Strong interpersonal, decision-making, problem-solving, oral and written skills.

Responsibilities

• Provide housing-focused case management to households in the Supportive Housing Program.
• Create and maintain accurate case files and case records.
• Develop housing stabilization service plans collaboratively with participants and track progress toward goals.
• Coordinate services with community agencies to address housing barriers and complex needs.
• Assist participants with developing long-term permanent housing goals and strengthening community networks.
• Coordinate with local social service agencies, health departments, and medical centers to ensure services are provided.
