Commonwealth Catholic Charities in Richmond, VA is seeking an Experienced Case Manager to provide housing-focused services to individuals experiencing chronic homelessness, including those with addictions or mental illness. You will develop and monitor housing stability plans, coordinate with community partners, and help participants access resources.

The role requires a Bachelor's Degree and at least two years of related experience, plus a valid driver's license.

Qualifications

• Bachelor's Degree required with two years of related experience or equivalent education and experience.
• Must have a valid driver's license

Responsibilities

• Provide housing-focused case management to households participating in the services offered by Supportive Housing Program.
• Prepares and maintains complete and accurate case files and case records.
• Creates housing stabilization service plans collaboratively with program participants and ensures progress towards the participants' housing and financial goals.
• Coordinate services with appropriate community agencies to address housing barriers and complex needs of participants.
• Assist participants with developing long-term permanent housing goals and strengthening community networks.
• Collaborates with landlords and other housing liaisons to seek out cost effective rental opportunities for clients.
• Negotiates with landlords and other housing liaisons to reduce fees and/or avoid evictions.
• Ensures completion of appropriate safety inspections for client housing opportunities.
• Enter all appropriate information into agency database in a timely manner.
• Creates and distributes all required reports in a timely manner and as directed by management.
• Represents the Agency in the community and workplace in a professional and ethical manner.
• Demonstrates sensitivity to the service population’s cultural and socioeconomic characteristics.
• Demonstrates strong interpersonal, decision-making, problem-solving, oral, and written skills.
• Utilizes a basic knowledge of Microsoft Office applications.
