Job summary

Catholic Community Services (CCS) Nativity House Apartments seeks a Housing Stability Case Manager to support individuals and families facing chronic homelessness. The role uses a housing-first, trauma-informed, strengths-based approach to promote self-sufficiency and well-being.

Responsibilities include comprehensive assessments, care coordination, crisis intervention, and collaboration with property management and clinical teams to ensure safe and stable housing for residents.

Qualifications

• Experience providing housing stability or case management services.
• Knowledge of community resources and social service systems.
• Proficiency with basic computer applications and databases.

Responsibilities

• Case Management & Service Delivery: conduct assessments and develop Housing Stability Plans with measurable goals.
• Provide ongoing, individualized case management to residents in permanent supportive housing.
• Assist clients in accessing healthcare, mental health services, benefits, and employment resources.
• Maintain regular contact with clients via home visits, office meetings, and field outreach.
• Conduct financial coaching to help clients budget for independent living.
